Consent forms / Clinical information form.
We CAN NOT proceed with genetic analysis without the consent form signed.
English Speaking Participants
- If the participant speaks English you must use the long consent form.
Non-English Speaking Participants
- Short consent forms are acceptable if the participant does not speak English. If the participant's language is not available as one of the consent forms,
you may translate it (e-mail us if you need the English short consent form to be used as a template
for translation.) The new translation must be approved by our IRB committee before proceeding with blood extraction.
- You will notice that when you download a short consent form, you will also download the English long consent form (to be used as a summary of the research protocol). The investigator has to
explain to the participant the information indicated in the short consent form in a language understandable to him in front of a witness.
- The participant and the witness must sign the short consent form.
- The person obtaining consent and the witness must sign the long consent form.
- Please give a copy of both documents to the participant and enclose with the blood package.
Please include in the package the clinical information
form and a copy of the tests that you consider necessary, ECG, EPS, etc.
